Boeing 737 Airplanes with Collins Aerospace SVO-730 Rudder Rollout Guidance Actuators - NTSB URGENT

 



The National Transportation Safety Board (NTSB) is providing the following information to urge The Boeing Company and the Federal Aviation Administration (FAA) to take immediate action on the safety recommendations in this report concerning the potential for a jammed or restricted rudder control system on certain Boeing 737 airplanes. We identified these issues during our ongoing investigation of the rudder pedal anomaly involving a Boeing 737-8, N47280, while landing at Newark Liberty International Airport (EWR), Newark, New Jersey, on February 6, 2024.

 Federal Aviation Administration:

  Determine whether Collins Aerospace SVO-730 rudder rollout guidance actuators with incorrectly assembled bearings should be removed from Boeing 737NG and 737MAX airplanes and, if so, direct US operators to remove the actuators until acceptable replacement actuators become available for installation. (A-24-29) (Urgent)

If you determine the Collins Aerospace SVO-730 rudder rollout guidance actuators with incorrectly assembled bearings should be removed, notify international regulators that oversee operators of Boeing 737 airplanes about the safety issues involving the SVO-730 rudder rollout guidance actuator and encourage them to require the removal of actuators with incorrectly assembled bearings from 737NG and 737MAX airplanes until an acceptable replacement actuator becomes available for installation. Aviation Investigation Report AIR-24-06

 Background and Analysis

On February 6, 2024, about 1555 eastern standard time, the flight crew of United Airlines flight 1539, a Boeing 737-8, N47280, experienced a rudder pedal.

 anomaly while landing at EWR.1 In a postincident statement, the captain reported that, during the landing rollout, the rudder pedals were “stuck” in their neutral position and did not move in response to the “normal” application of foot pressure to maintain alignment with the runway centerline.2 The flight was operating under the provisions of Title 14 Code of Federal Regulations Part 121 as a scheduled international passenger flight from Lynden Pindling International Airport, Nassau, Bahamas, to EWR.3

According to data derived from the flight data recorder, the flight crew applied approximately 32 pounds of force to the rudder pedals before touchdown which yielded no discernible effect on the rudder position or heading.4 The flight crew attempted to clear the jammed rudder controls immediately after touchdown, applying approximately 75 pounds of force to the rudder pedals when the airspeed was about 120 knots, again with no effect on the rudder position or heading.

With the airplane’s airspeed continuing to decrease during rollout, the flight crew applied approximately 42 pounds of force to the pedals, but the jam persisted. The captain elected instead to use the nosewheel steering tiller as the airplane slowed to a safe taxi speed. The captain stated that, after the airplane entered the assigned taxiway, he asked the first officer to check the rudder pedals on his side of the flight deck, and the first officer indicated that the same anomaly was occurring.

Data derived from the flight data recorder indicate that shortly after, with the airplane traveling at a groundspeed of less than 20 knots, the flight crew applied approximately 59 pounds of force on the rudder pedals, and the rudder pedals and rudder surface began to operate normally. The airplane taxied to the gate without further incident, and all airplane occupants (2 flight crewmembers, 4 cabin crewmembers, and 155 passengers) deplaned without any injuries or damage to the airplane.

United Airlines received the incident airplane from Boeing on February 20, 2023. The airplane was equipped with a Collins Aerospace SVO-730 rudder rollout guidance actuator, which was electrically disabled based on the operator’s delivery requirements for the autoflight system.5 Although the actuator was disabled, it remained mechanically connected to the upper portion of the airplane’s aft rudder input torque tube by the actuator’s output crank arm and a pushrod, as shown in figure 1.

 The Collins SVO-730 rudder rollout guidance actuator is installed only on Boeing 737NG and 737MAX airplanes equipped for category IIIB operations. (The incident 737-8 was a MAX variant.) United Airlines does not require category IIIB capability for its Boeing 737 fleet. According to FAA Advisory Circular 120-28D, category IIIB operations involve a precision instrument approach and landing with no decision height and a runway visual range less than 700 ft but not less than 150 ft.

6 Pilot control of the Boeing 737-8 rudder is transmitted in a closed-loop system from the pilots’ rudder pedals in the cockpit, through a single cable system, an aft rudder quadrant, and a pedal force transducer, to the aft rudder input torque tube in the vertical stabilizer. Rotation of the torque tube provides the command inputs to the main and standby rudder power control units to move the rudder surface.

PICKLE FORKS CRACKS - On youth fleet of B737 NG - Severe vibration incident on a Boeing Model 737-800 series airplane

 


The aircraft, registered B-1791, was 6.8 years old and completed 8,986 flight cycles. It had not met the ‘pickle fork’ inspection threshold of 22,600 flight cycles.

Cracks found in critical part (called a pickle fork) that keeps wing attached to 737-NG (not MAX) fuselage. One found earlier in month, now more on other planes. Found early in plane's lifespan. Boeing & FAA scrambling to find extent of problem.

Hairline cracks were found in the pickle forks of Boeing’s 737 New Generation family, which includes the 737-800, in 2019.

The FAA will required operators on certain Being 737 NG jetliners to conduct inspections for structural cracks. Boeing notified the agency of the matter after it discovered the cracks while conducting modifications on a heavily used aircraft. Subsequent inspections uncovered similar cracks in a small number of additional planes. The FAA have instructed operators to conduct specific inspections, make any necessary repairs and to report their findings to the agency immediately.

Pickle Forks Cracks

Named after the utensil used to pierce and handle pickles, pickle forks are the component that attach the plane’s body to its wing structure. These components help to “manage the stress, torque and aerodynamic forces that bend the connection between the wings and the body of the jet”.

It is not clear yet what causes the cracks in the forged aluminum part. Many older NG were retrofitted with winglets on the tips of their wings. These may have led to unforeseen loads or vibrations. It is possible that some of the younger 737 NG airplanes have a similar problem.

SUPPLEMENTARY INFORMATION: On June 2, 1999, the FAA received a report of a severe vibration incident on a Boeing Model 737-800 series airplane, which had accumulated 3,517 total flight hours and 1,284 total flight cycles. The airplane was involved in a high-speed descent with speed brakes extended while operating at an airspeed of 320 knots. During the descent, severe vibration occurred at 250 knots. At 230 knots, the speed brakes were retracted and the vibration stopped. The landing was uneventful.

Inspection of the airplane revealed that the upper flange of the right elevator tab mast fitting, to which the elevator tab push rods are attached, was found fractured. The lower flange of the fitting was not damaged. In addition, excessive freeplay in the elevator tab also was observed and measured during the inspection.

Further analysis confirmed that the damage to the fitting was aggravated by speed-brake-induced airframe vibration. Such vibration could lead to damage of the elevator tab mast fitting, excessive freeplay in the tab, and consequent separation of the tab mast fitting from the tab. Excessive freeplay in the tab could result in severe airframe vibration and consequent damage to the tab, elevator, and horizontal stabilizer. Separation of the elevator tab mast fitting will result in a free tab. These conditions, if not corrected, could result in loss of controllability of the airplane.

The flight data recorder was found around 9.20am buried about 1.5 meters (5 feet) under the soil at the mountainous crash site in the southern Chinese region of Guangxi, the report said. “It was cylindrical and covered in soil,” Chen Xiaohui, the deputy commander of the fire brigade in the regional capital Nanning, told CCTV.

The device will be flown to Beijing for data analysis alongside the cockpit voice recorder, which was found on Wednesday. The flight data recorder stores important data such as the flight path, speed, altitude and engine power, which can reveal whether human error or instrument problems could have contributed to a crash. Searchers had been using metal detectors to look for the device in hilly countryside near Wuzhou city.

CAE Hurry Up Production of Boeing 737 Max Flight Simulators

Long a manufacturer of full-flight simulators, CAE also runs the world’s largest training network for civil aviation pilots, with about 300 devices deployed at company-run facilities in cities such as Abu Dhabi, Kuala Lumpur and London. CAE says its 2,000 instructors train more than 135,000 pilots every year.
Airlines that bought the MAX have had to cancel flights while the global fleet remains grounded.
If only computer-based training were required for renewed 737 MAX commercial flying, Boeing’s largest customer Southwest Airlines Co had said it would take one to two months to prepare its more than 9,500 pilots on the updates.
Southwest had not been part of Boeing’s recent discussions on pilot training recommendations and could not provide additional cost or timing estimates before there was specific guidance, spokeswoman Brandy King said.
Southwest has three MAX simulators in various stages of FAA certification and expects three more later this year.
American Airlines Group Inc and United Airlines each have one MAX simulator.
Not many MAX simulators exist and it was unclear if training could be performed on the 737 NG simulator that 737 pilots have used until now. As of December, Toronto-based manufacturer CAE Inc had delivered 23 MAX full-flight simulators, a spokeswoman said.
The FAA said on Tuesday it will consider Boeing’s recommendations during training evaluations of U.S. and international flight crew alongside regulators from Canada, Europe and Brazil.
Canada’s Transport Minister Marc Garneau, who has leaned toward mandatory simulator training for Canadian 737 MAX operators, said in a statement that Boeing’s signaling the importance of such training was “encouraging.”

Boeing 737's Horizontal Stabilizer Must Be Scrutinized



FAA: This AD is effective May 20, 2013.

We are adopting a new airworthiness directive (AD) for all The Boeing Company Model 737-600, -700, -700C, -800, -900, and -900ER series airplanes. This AD was prompted by reports of an incorrect procedure used to apply the wear and corrosion protective surface coating to attach pins of the horizontal stabilizer rear spar. This AD requires inspecting to determine the part number of the attach pins of the horizontal stabilizer rear spar, and replacing certain attach pins with new, improved attach pins. We are issuing this AD to prevent premature failure of the attach pins, which could cause reduced structural integrity of the horizontal stabilizer to fuselage attachment, resulting in loss of control of the airplane.
 

We estimate that this AD affects 1,050 airplanes of U.S. registry.

 Applicability

(1) This AD applies to all The Boeing Company Model 737-600, -700, -700C, -800, -900, and -900ER series airplanes, certificated in any category.

(2) Installation of Supplemental Type Certificate (STC) ST00830SE (http://rgl.faa.gov/Regulatory_and_Guidance_Library/rgstc.nsf/0/408E012E008616A7862578880060456C?OpenDocument&Highlight=st00830se) does not affect the ability to accomplish the actions required by this AD. Therefore, for airplanes on which STC ST00830SE is installed, a “change in product” alternative method of compliance (AMOC) approval request is not necessary to comply with the requirements of 14 CFR 39.17. For all other AMOC requests, the operator must request approval for an AMOC in accordance with the procedures specified in paragraph (k) of this AD.

 

Unsafe Condition

This AD was prompted by reports of an incorrect procedure used to apply the wear and corrosion protective surface coating to attach pins of the horizontal stabilizer rear spar. We are issuing this AD to prevent premature failure of the attach pins, which could cause reduced structural integrity of the horizontal stabilizer to fuselage attachment, resulting in loss of control of the airplane.

Part Number (P/N) Inspection

For airplanes having line numbers 1 through 3534 inclusive: Before the accumulation of 56,000 total flight cycles, or within 3,000 flight cycles after the effective date of this AD, whichever occurs later, inspect to determine the part number of the attach pins of the horizontal stabilizer rear spar. A review of airplane maintenance records is acceptable in lieu of this inspection if the part number of the attach pin can be conclusively determined from that review.
 

Replacement

If, during the inspection required by paragraph (g) of this AD, any horizontal stabilizer rear spar attach pin has P/N 180A1612-3 or 180A1612-4, prior to the accumulation of 56,000 total flight cycles on the pin, or within 3,000 flight cycles after the effective date of this AD, whichever occurs later, replace with a new attach pin having P/N 180A1612-7 or 180A1612-8, respectively, in accordance with the Accomplishment Instructions of Boeing Special Attention Service Bulletin 737-55-1093, dated April 9, 2012.

Esta emergência foi resultada de um relatório indicando que um Modelo de  Boeing série 737-300, matrícula N632SW, experimentou uma descompressão rápida quando uma junção na cobertura do teto na corda S-4L entre a estação (BS) 664 e BS 727 rachou e abriu. A investigação mostrou que a rachadura foi localizada na superfície inferior da fileira de fixadores. A aeronave tinha acumulado 39.781 ciclos totais de voo e 48.740 horas totais de voo. Esta condição, se não corrigida, poderia resultar numa descompressão descontrolada no avião. The Southwest Airlines jet forced to land last week after a gash opened in its fuselage had made an average of seven flights a day over its 15 years of service - a demanding schedule for any jetliner.

Aviation experts had initially speculated that the wear and tear Southwest planes typically endure - making an average of six flights per day - contributed to the incident. Southwest is the leading low-cost carrier in an industry that is highly competitive.

After the incident, Southwest cancelled about 630 flights and inspected its 78 Boeing 737s, finding five others with fuselage cracks. The airline resumed normal flight operations Tuesday.

Also on Tuesday, the FAA ordered all airlines to conduct detailed inspections within five days of older model Boeing 737-300s, 400s and 500s that have logged more than 35,000 flight cycles. The directive also requires airlines to check heavily-used 737s before they reach 30,000 cycles and orders that the older planes, which are mostly owned by Southwest, be re-inspected every 500 cycles.

Paul Richter, chief project engineer for 737s at Boeing, said during a media briefing that the fatigue cracks suspected in the Southwest rupture occurred sooner than Boeing expected in the life of the plane. The company also issued a service order instructing airlines to check their planes.

Southwest Airlines Flight 812, a Boeing 737 carrying about 118 people out of Phoenix for Sacramento, Friday, diverted to Yuma Marine Corps air station in Arizona after an in-flight fuselage rupture caused rapid decompression at 36,000 feet. Passengers who called in to local news stations said a six foot long gash opened with a loud bang in the top of the cabin. They said the sky was visible through the opening and that some passengers lost consciousness during the rapid descent to 11,000 feet. According to Southwest, a flight attendant was the only one aboard to have suffered an injury. One passenger who saw the flight attendant speculated that the crewmember may have broken his nose. The NTSB has sent a Go Team to investigate the fuselage rupture.
